The locusts are given power to torment people who don't have God's seal for five months. The torment is so intense that people wish to die but can't. Death is denied them.
The five months might correspond to the lifespan of locusts. Or it might represent a specific duration of suffering. Either way, it's not eternal. But it feels endless to those enduring it.
The cruel element is that people want death as escape but are denied it. Hell often includes the inability to escape. The trapped feeling. The knowledge that relief won't come. That's a different kind of torment from physical pain.
