The fourth horseman is named Death, and Hades follows him. Together they're given authority over a quarter of the earth to kill by sword, famine, plague, and the wild beasts.
The pale horse suggests sickness, decay, the color of a corpse. Death is personified. Not accident or random. Purposeful movement. The endpoint of the progression. Conquest, war, famine, and now death on a massive scale.
A quarter of the earth dies. Not total destruction yet, but massive loss. The judgment is terrifying but measured. There's still three-quarters left. Still time. Still hope for repentance, though most won't take it.
